Overview

Groups is a rapidly growing, mission-driven healthcare company that provides evidence-based treatment for Opioid Use Disorder. Our model uses two of the most powerful tools for recovery: group therapy and medications including buprenorphine-naloxone. Our approach takes the best parts of mutual aid groups (support, and community) and combines them with the latest medical science and technology. At Groups, our medical providers are a key component of our multidisciplinary care team that also includes substance abuse counselors, office managers and peer recovery coaches. Our medical leadership provides a mixture of direct member care and administrative/leadership responsibilities.

At Groups, the Site Medical Director has both clinical and administrative responsibilities.

Clinically, you will work on a team of medical providers, counselors, and administrators to provide direct care and medication management for our members throughout their treatment course. You will leverage telemedicine extensively in your work, so must be comfortable with technology and enthusiastic about the benefits of this modality. You will work daily to overcome the barriers faced by members early in recovery. This will require you to adopt a member-first attitude that prioritizes keeping members safe, by supporting ongoing engagement in treatment and continuous access to medications. Flexibility, empathy, and commitment to member success are critical attributes for a successful medical provider. You will support your colleagues across the care team disciplines by providing support when medical questions or needs arise and coverage when other providers are unavailable.

As medical leader, you will directly manage medical providers and be responsible for overall quality of care and ensuring that the medical treatment provided is consistent with Recover Together’s policies and procedures and the Groups Recovery System Manual. This includes collaborating with HR and the Provider Support Manager to on-board new providers as well as completing annual evaluations and performance management of existing providers assigned to you.

Additional administrative/leadership duties will include: Serving as the backstop for all medical concerns, serving (or delegating) as needed as the collaborating/supervising physician for NPs and PAs (where applicable), performing direct observation and chart review of other providers to ensure quality, serving as an expert consultant for complex individual clinical cases and interdisciplinary care team meetings, maintaining high quality care through performance management of providers assigned to you and working with the regional leadership team in developing and deploying quality improvement initiatives.

The Site Medical Director duties are designed to ensure that the care being delivered at each of our sites is of the highest quality and consistency, and meet the requirements of the state for this role. The Site Medical Director will manage a small team of providers at designated sites, monitoring quality, reporting to regional leadership any concerns, and helping to take action to improve the quality of care we deliver. This role reports to the State Medical Director. The Site Medical Director will also serve as a medical care provider to members at designated groups each week.

Responsibilities

Essential Functions:

Clinical (50-75% time, depending on # sites covered and hours per week):

  • Adhere to all clinical policies and procedures as outlined in the GRS Manual.
  • Direct member care duties
  • Determine each member’s medical necessity and appropriateness for treatment pursuant to applicable clinical models, policies and procedures, and manuals (e.g., Recovery System Manual) as required by Groups Recover Together.

  • Prescribe medications for opioid use disorder and, where appropriate other approved medications, in a manner consistent with Recover Together’s policies and procedures and the Groups Recovery System Manual.

  • Maintain a panel of patients and serve as their dedicated provider, reviewing their care plan and medication management at each medical visit, or more often as necessary

  • See members in individual sessions to review medical progress and provide E&M services, when necessary.

  • Sign all prescriptions on the same business day that they are generated.
  • Coordinate with community providers or agencies to support members, as necessary.
  • Provide, as needed, cross-coverage for colleagues and answers to medical questions when other providers are unavailable or clinical cases are escalated for your review.
  • Serve as medical consultant for interdisciplinary care team meetings.

  • Participate in the rotating “on-call” schedule of urgent prescription coverage.

  • Facilitate access to the State’s prescription drug monitoring database so Groups staff can regularly monitor the members access to controlled substances, per Recover Together’s clinical protocols.

  • Complete high quality, thorough and accurate documentation for all medical encounters on the same business day as the encounter and in accordance with policies of the company.

  • Maintain patient records in confidence and complete records as required by HIPAA and other applicable federal/state laws, rules or regulations/company policies regarding medical records.

Administrative / Management (25-50% time, depending on # sites covered and hours per week):

  • Directly manage medical providers assigned to you; you’ll be responsible for ensuring the overall quality of care and medical treatment is consistent with Groups’ policies and procedures and the Groups Recovery System Manual.

  • Be an expert on Groups clinical and operational policies, procedures, and workflows.

  • Be able to navigate complex clinical situations and “gray areas” in a manner consistent with Groups’ philosophy and harm reduction focused culture.

  • Serve as the primary backstop for all medical concerns; be available for day to day escalations.

  • Assist with onboarding, training, and support of new and existing medical providers.

  • Perform periodic chart reviews and direct observation of care for assigned providers and follow up with feedback, performance management, and associated documentation.

  • Assist with roll-out of new protocols, workflows, and care pathways.

  • Partner with care delivery leaders on continuous improvement of medical/care team service delivery.

  • Provide essential middle management of the front-line providers (communication, advocacy, feedback, filtering, etc).

  • Work with the medical leadership team to pilot new initiatives aimed at improving clinical outcomes and experience.

  • Serve as collaborating physician for assigned NP/PA providers, if applicable. This includes completing supervision, documentation reviews, and chart reviews, as required by state specific.

Site Medical Director Responsibilities:

  1. Visit the physical site each week

    1. Be present at each site for 25% of the time each site is open for services each week, with a minimum of 5 hours per site per week.

    2. During this time onsite, the SMD may conduct group therapy, conduct in-person visits, perform monthly chart audits, meet with staff, or other GRT-related activities and duties.

  2. Provide chart oversight

    1. Review and sign the designated percentage of charts at the site per month. (10% of the site’s census monthly)

    2. Document chart audit in GRT’s chart audit form.

    3. Offer feedback and management to providers and other staff re: findings from chart audits to help the team improve quality.

  3. Provide collaborative agreements and oversight to site providers

    1. Provide collaborative oversight to the NP/PAs who work at that site as needed.

    2. Follow state guidelines on frequency of consultation, observation, and chart audits for those NP/PAs, and document as appropriate.

    3. Deliver feedback and support drawn from group observations and chart audits to site providers.

  4. Advise and take action to support quality improvement

    1. Attend twice-yearly Quality Committee meetings to report out on quality and medical matters and learn about relevant information in your state.

    2. Discuss trends and observations with the Regional Medical Director and staff.

    3. Lead or support ongoing Quality Improvements projects at your sites.

  5. Meet core state regulatory requirements

    1. Serve as named "Site Medical Director" on necessary licensing, credentialing, and certification applications.

    2. Be available and accessible for all payer and state audits.

    3. Serve as Site Medical Director for no more than 3 sites in the state.

Qualifications

Knowledge, Skills and Abilities:

  • Ability to demonstrate recovery-sensitive behaviors, including choice of language, attitudes and interactions with members and staff.

  • Ability to thrive in the group medical visit setting and use telehealth modality effectively.

  • A commitment to harm reduction principles and ability drive a culture of member centeredness and harm reduction on the teams you supervise.

  • Ability to maintain courtesy and professionalism in all communications (verbal and written) with staff and members.

  • Ability to use discretion and work independently under general supervision.

  • Ability to understand and adhere to the Professional Code of Conduct.

  • EMR fluency, ideally with eClinical-Works

  • Spreadsheet and basic data analysis proficiency

  • Physician with active medical license and appropriate behavioral health or addiction medicine NPI taxonomy (or willingness to adjust primary or secondary NPI taxonomy).

  • Addiction Medicine board certified or board eligible strongly preferred.

  • At least 5 years of medical leadership or management experience either during or after residency training.

  • Active medical license in good standing in a state where Groups Recover Together operates and willingness and ability to obtain additional licenses in other states (no background issues that could limit state licensing or credentialing).

  • Membership in the Interstate Medical Licensing Compact or willingness to join.

  • Willingness and ability to become credentialed with all government and private health plans with whom Groups Recover Together is in network.

  • At least 1 year of experience managing OUD with buprenorphine using a low-barrier philosophy.

  • At least 1 year experience working in a team-based-care setting with behavioral health providers (therapists, social workers, case managers, etc).

  • At least 2 years experience working with a similar member population (primarily medicaid, high burden of SDOH, SUD, and mental health diagnoses).

  • Access to reliable internet and telephone services, specifically 10M download and 5M upload packages or higher, as well as a strong WiFi signal from your remote work location.

  • Must meet pre-employment requirements and maintain all applicable state and job-related guidelines for background screening. Depending on state-specific requirements, this may include fingerprinting, drug testing, health screening, COVID vaccination verification, CPR/Basic First Aid and license/credential verifications.
