The Patient Flow Coordinator is responsible for the efficient, timely coordination of health care services provided to numerous patient populations within VCUHS and the community. This position supports and coordinates all patient placement activities within the facility and the community through the One Call Center.
The Patient Flow Coordinator consistently anticipates and critically thinks through problems/potential issues and takes initiatives to resolve with minimal direction. The role adheres to department policies and procedures. The Patient Flow Coordinator monitors compliance of the policies and procedures governing patient placement, EMTALA regulations, and registration procedures.
Licensure, Certification, or Registration Requirements for Hire: Current RN licensure in Virginia or eligible or compact Licensure, Certification, or Registration Requirements for continued employment: Current RN licensure in Virginia or Compact State
Experience REQUIRED:
Five (5) years clinical experience in an acute care setting Two (2) year of nursing management experience
Experience PREFERRED:
Academic healthcare experience Clinical triage experience ICU or ED experience
Education/training REQUIRED:
Baccalaureate Degree in Nursing from an accredited School of Nursing
Education/training PREFERRED:
N/A Independent action(s) required: Independent decisions include interpretation of criteria for specific case application. Works autonomously and in concert with others to make patient placement decisions. Resolves conflicts independently and if unable to do so, identifies and utilizes appropriate resources and escalates appropriately. Prioritization of daily activities and collaboration with interdisciplinary healthcare team.
Supervisory responsibilities (if applicable): N/A
Additional position requirements:
RN Patient Flow Coordinator works all shifts. Schedules, including requirements of shift rotations and hours of work may be adjusted with 72 hour notice as necessary to meet staffing requirements or as is in the best interest of the work department.
Age Specific groups served:
All Physical Requirements (includes use of assistance devices as appropriate): Physical Lifting 20-50 lbs. Activities: Prolonged sitting, Walking (distance)
Mental/Sensory:
Strong recall, Reasoning, Problem solving, Hearing, Speak clearly, Write legibly, Reading, Logical thinking
Emotional:
Fast pace environment, Steady pace, Able to handle multiple priorities, Frequent and intense customer interactions, Noisy environment, Able to adapt to frequent change
